Chiropractic Care in Each Trimester: What to Expect
Pregnancy transforms your body in profound ways — and those changes don’t happen all at once. Each trimester brings a new set of physical demands, postural shifts, and hormonal changes that affect your spine, pelvis, and nervous system. Chiropractic care can be a valuable source of support throughout all three stages — but what that care looks like evolves as your pregnancy progresses.
Here’s what to expect at each stage when you visit West University Wellness.
First Trimester: Laying the Foundation (Weeks 1–13)
The first trimester is often when women feel the most uncertain about chiropractic care. The good news: gentle chiropractic adjustments are generally considered safe during the first trimester and may be especially beneficial during this foundational period.
During these early weeks, your body begins producing relaxin — a hormone that loosens ligaments throughout the pelvis in preparation for birth. While this is a natural and necessary process, it can also create instability and discomfort, particularly in the sacroiliac joints and lower back.
In the first trimester, our focus is on:
• Establishing a baseline assessment of your spinal alignment and pelvic balance
• Addressing any pre-existing subluxations before they are compounded by pregnancy changes
• Supporting nervous system function as your body undergoes rapid hormonal shifts
• Reducing early low back, hip, and neck tension
Adjustments in this trimester are gentle and do not involve any pressure on the abdomen. Many patients find they also experience relief from headaches and nausea as a secondary benefit of improved spinal alignment.
Second Trimester: Supporting a Growing Body (Weeks 14–26)
The second trimester is often called the “honeymoon phase” of pregnancy — energy returns, nausea typically subsides, and the baby bump begins to show. But as your center of gravity shifts forward, your lumbar spine takes on new stress, and round ligament pain becomes a common complaint.
This is also the stage when fetal positioning becomes increasingly important. At West University Wellness, we begin paying close attention to pelvic symmetry and intrauterine constraint during the second trimester — giving your baby the best possible environment to move and grow.
In the second trimester, our approach includes:
• Webster Technique adjustments to maintain pelvic balance as your uterus expands
• Soft tissue work on the round ligaments to reduce tension and discomfort
• Postural support and ergonomic guidance as your center of gravity shifts
• Use of pregnancy pillows and table modifications for your comfort during visits
Third Trimester: Preparing for Birth (Weeks 27–40+)
The third trimester is when chiropractic care becomes most time-sensitive. As your baby grows and descends, pelvic alignment plays a direct role in how much room your baby has to position optimally for labor. This is also when many mothers experience the most discomfort — including sciatica, pubic symphysis pain, and significant low back pressure.
We modify our techniques significantly in the third trimester. We use the Thompson drop table — a specialized chiropractic table that allows for safe, effective adjustments without any pressure on your abdomen — making it ideal for women in their late pregnancy.
In the third trimester, our priorities are:
• Maintaining optimal pelvic alignment to support fetal positioning
• Reducing sacral subluxation and intrauterine constraint
• Managing sciatic pain, pubic symphysis discomfort, and rib tension
• Labor preparation adjustments starting at 40 weeks, which may help support the body’s readiness for birth
How Often Should You Come In?
Every pregnancy is different, and we tailor visit frequency to your individual needs. As a general guide, many of our patients benefit from:
• First trimester: once or twice a month, or as needed for discomfort
• Second trimester: every two to three weeks as postural changes increase
• Third trimester: weekly, especially from 36 weeks onward as you approach your due date
We will always work alongside your OB-GYN or midwife to ensure your care plan is coordinated and appropriate for your specific pregnancy.
